Certificate of Production Insurance: If you are renting equipment as part of a production, we will need to have a Certificate of Insurance with ‘Wintech Video’ as Additionally Insured and Loss Payee. If you do not have production insurance, we will need to place a hold on a credit card for the replacement value of the equipment.

Credit Card Authorization Form: This will be kept on file for future reference and for Payment Processing. Please download this form HERE. You can either scan and email the signed form to wintechvideo@gmail.com or fax it to (818) 501-6566. Please include a copy of the Card-holder’s ID.

Account Information: Please fill out the fields below to begin the process of opening an account with us. Please include references from other rental houses you may have used in the past and/or who referred you to us.
